Introduced by the Government of Ontario in February 2019, Ontario Health Teams (OHTs) are a new model of health care delivery that puts patients/clients, families and caregivers at the centre of the health care system. OHTs are intended to improve the coordination of care and services for people and their families/caregivers across multiple care settings.
The vision is to better connect different parts of the health care system by bringing together, as one team, patients/clients, families, communities, providers and system leaders to deliver more coordinated care for people in a defined geographic area. The members of the OHT, even if in different organizations or physical locations, work together toward common goals for improving access to, and delivery of, local health care services.
The Hastings Prince Edward Ontario Heath Team (HPE OHT) is a partnership of health and human service care providers from Hastings County, Prince Edward County and the Tyendinaga Mohawk Territory, working in the shared purpose of “Caring for our communities together. Partnering for better health and wellness, within, and across our communities.” The Hastings Prince Edward Ontario Health Team (HPE OHT) would like to hear from residents of Hastings County or Prince Edward County who do not have a designated primary care physician or nurse practitioner in our region.
A better understanding of these experiences with healthcare will help inform discussions and support decision-making as the OHT works to transform health care and human services in HPE. Connecting with those who have diverse lived experiences will be critical to the success of this initiative.
If you might be interested in participating in a brief interview to share thoughts and experiences, are asked to contact Stacey Hatch at [email protected] by March 15, 2023. The HPE OHT provides critical health and human services for an area spanning from the shores of Lake Ontario and the Bay of Quinte to Algonquin Provincial Park and beyond.
